Medway serious case reviews

The main purpose of a Serious Case Review (SCR) is for agencies and individuals to learn lessons to improve the way they work both individually and collectively to safeguard and promote the welfare of children.

It is not an inquiry into how a child died or was seriously harmed, or who is culpable. These are matters for coroners and criminal courts to determine as appropriate.

Under Regulation 5 of The Local Safeguarding Children Board Regulations 2006, a Local Safeguarding Children Board (LSCB) must undertake a SCR if:

  • abuse or neglect of a child is known or suspected
  • the child has died, or been seriously harmed and there is concern as to how the authority, their board partners or other relevant persons have worked together to safeguard the child

A SCR is led by an individual who is independent of the LSCB and the organisations involved in the case.

Medway Safeguarding Children Board (MSCB) has published a serious case review in response to the identification of abuse of children by staff at the Medway Secure Training Centre (STC) in a television documentary broadcast in January 2016.
